Placement Test Dates for the New Trier Class of 2031

Saturday, November 7^th and Saturday, November 14^th

Test registration for Township students will be in October 2026

Class of 2030 - Health Services Welcome Letter
IL Certificate of Child Health Examination Form – for class of 2030: school physical completed on/after 8/20/2025
IL Proof of Dental Examination Exam Form – by May 15th of their freshman year, students in grade 9 are required to have a dental exam on file. School dental examinations must have been completed within 18 months prior to the May 15 deadline. For the class of 2029, dental examinations completed on/after November 15, 2024.
Dental Examination Waiver Form – if unable to have exam due to lack of/insufficient coverage
NTHS Medication Form – required for all prescription and over-the-counter medications including ibuprofen, acetaminophen, and diphenhydramine
Allergy Action Plan – for students with potential anaphylactic allergies
Asthma Action Plan – for students with asthma
Seizure Action Plan – for students with known seizure history or epilepsy
Diabetes Medical Management Plan – for students with diagnosis of diabetes
