- IL Certificate of Child Health Examination Form – Class of 2029: school physical completed on/after 8/20/2024
- IL Proof of Dental Examination Exam Form – Class of 2029: dental examination completed on/after November 15, 2024.
- Dental Examination Waiver Form – if unable to have exam due to lack of/insufficient coverage
- NTHS Medication Form – required for all prescription and over-the-counter medications including ibuprofen, acetaminophen, and diphenhydramine
- Allergy Action Plan – for students with potential anaphylactic allergies
- Asthma Action Plan – for students with asthma
- Seizure Action Plan – for students with known seizure history or epilepsy
- Diabetes Medical Management Plan – for students with diagnosis of diabetes
